Insurances
Lacrosse Victoria and its members are covered by a number of
insurances. Details are available within the downloads below:
- Match Day
Accident Report Form
This form must be completed by the Game Day Bench Officials for
any injury during a games and submitted to Lacrosse Victoria with
the score sheet.
- Personal Injury
Report Form
This form must be completed by any player who sustains a
significant injury. Lodgement of this form with Lacrosse Victoria
activates a claim with our insurers.
Risk Management and Forms
The following Risk Management Forms are made available for club
administrators:
- Emergency
Telephone Numbers
This form must be displayed inside all lacrosse clubrooms. Please
make sure each appropriate area is filled in
- Match Day
Preparation Report
This form must be completed by each club prior to a day's play.
Please ensure the form is kept in a secure location
- Team Ground
Inspection Report
This form must be filled in by a home club representative, an away
club representative (captain, coach, manager) as well as a
referee/umpire before the commencement of the game. To be forwarded
by the home team with the match report
